If you wish to register with the practice and live within our practice boundaries please download the below New Patient Questionnaire and fill out all the sections of the template.
You will be registered with the practice but have the right to choose a GP. You will need to complete the new patient questionnaire (basic information about yourself and your medical history) and should you wish to have a health check please make an appointment with the healthcare assistant. (The check consists of checking your blood pressure, height, weight, and medical history).
Children under five years of age can be registered for child health surveillance. In addition you will be asked to provide details of childhood immunisations of children under six (this will ensure your child is included in the computerised recall system). Medical treatment is available from the date of registration. Please contact reception for further information.

- M3 – City Centre, Deansgate, Castlefield (Not Salford)
- M4 – Ancoats, Northern Quarter
- M14 – Moss Side, Fallowfield( MMU Students)
- M15 – Hulme
- M16 – Moss Side, Whalley Range (Not Trafford)
If you do not live in any of the above postcodes then unfortunately we will not be able to accept your registration. Fear not, there are other Practices that will accept you. For your convenience you can follow this link to find a GP surgery near you
New Patient Registration Form
Once you have downloaded our registration form below which you can fill out in the comfort of your own home and either deliver to the practice or email it to us at [email protected].
Please be aware, when emailing the completed registration form we will need to to see photo identification with you stood next to the ID. We will also need to see proof of address, this could be a utility bill, council tax or tenancy agreement. Your registration form will not be processed without this information.
The Health Authority will confirm your registration within 6-8 weeks although you can book an appointment after your registration form has been processed. This will automatically be done. Please note, the practice will not contact you to confirm that your registration has been processed.
Once this has been completed you will be registered within a 7 to 10 working day period. If you have repeat medication, you will be required to make a GP appointment before any prescriptions are issued. This appointment is necessary as it gives us vital information as it can sometimes take a while for your original records to arrive from your previous GP.
